She was born into a German-speaking family in Zagreb, which was then part of the Austro-Hungarian Empire. She studied acting and began her career in the German-speaking theater in Zagreb. She worked in theaters all around Europe and became a well-known actress in her time.

Hertha von Hagen was known for her talent in portraying passionate and strong-willed women characters. In 1914, she appeared in the Austrian silent film, "Der Mann im Keller" ("The Man in the Cellar"), which was based on a play by Leopold Wohl. Von Hagen also appeared in four German films during the silent era, including "Das graue Haus" ("The Gray House") in 1926.
